Our transition services ensure that those individuals can be closer to their families and more engaged in their communities—allowing them to live full and meaningful lives.

Support Coordination
Support Coordination Services identify, coordinate, and oversee the delivery of services and supports to enhance the health, safety, and general well-being of individuals with IDD.

Intensive Support Coordination
Carefully designed for for individuals with IDD with exceptional medical and behavioral needs who need an additional level of services, intensive support coordination provides specialized coordination of waiver services and medical and behavioral support services.
