Moving From Institutions into the Community
DBHDD is committed to helping people live independently in their communities, regardless of their disabilities, while providing easy access to the high-quality care that they need to stay safe and healthy. As part of this commitment, DBHDD has facilitated many successful transitions from institutional care to community-based care for individuals with intellectual and developmental disabilities. Our transition services ensure that those individuals can be closer to their families and more engaged in their communities—allowing them to live full and meaningful lives.
