DBHDD, in partnership with Sangha Unity Network (SUN), will continue the work of Transformational Change through a series of interactive, one-day workshops focused on creating more meaningful Individual Service Plans using the new ISP template.

These workshops are designed to support a shift in thinking about the ISP, from a required document to a person-centered tool that reflects the aspirations, strengths, preferences, and choices of each person receiving services.

Participants will learn how to facilitate and develop ISPs that are grounded in person-centered engagement and aligned with the intent of the HCBS Settings Rule and NOW/COMP waivers.

This work reflects DBHDD’s ongoing commitment to practices that support autonomy, dignity, and meaningful community life.

These sessions are interactive and practice-based. Participants will engage in discussion, reflection, and applied learning activities to build confidence and consistency in implementing the new ISP template.
