DESCRIPTION:DBHDD\, in partnership with Sangha Unity Network (SUN)\, will c
 ontinue the work of Transformational Change through a series of interactiv
 e\, one-day workshops focused on creating more meaningful Individual Servi
 ce Plans using the new ISP template.\n\nThese workshops are designed to su
 pport a shift in thinking about the ISP\, from a required document to a pe
 rson-centered tool that reflects the aspirations\, strengths\, preferences
 \, and choices of each person receiving services.\n\nParticipants will lea
 rn how to facilitate and develop ISPs that are grounded in person-centered
  engagement and aligned with the intent of the HCBS Settings Rule and NOW/
 COMP waivers.\n\nThis work reflects DBHDD’s ongoing commitment to practi
 ces that support autonomy\, dignity\, and meaningful community life.\n\nTh
 ese sessions are interactive and practice-based. Participants will engage 
 in discussion\, reflection\, and applied learning activities to build conf
