Office of Constituent Services

The DBHDD Office of Constituent Services serves as a point of contact for the people we serve and their families, advocates, legislators, and the general public. We are here to answer questions, address complaints, and help you access DBHDD services. Our goal is to be accessible and responsive to your needs and identify areas that need improvement.

Confidentiality/HIPAA

It is the policy of DBHDD to ensure compliance with applicable state and federal laws and regulations regarding confidentiality and privacy of a person’s health care information, including information about mental health, developmental disabilities, or substance use disorders.  

 

Authorization for Release of Information

If you receive DBHDD services, or are the parent or legal custodian of a minor, or the guardian of someone receiving services, you may authorize us to access records and treatment information needed to help us assist you by completing and signing the Authorization for Release of Information Form.

Elected Officials

We are happy to assist your constituents directly and simply report to you that we have done so, without disclosing confidential information to you. Alternatively, if the person receiving services (or the parent or legal custodian of a minor, or a guardian) authorizes disclosures to you, please ask them to complete and sign the authorization form and submit it to our office.
